You should know if you are at risk for diabetes. Why? Studies show that people who are at risk for Type 2 diabetes can prevent or delay the onset of the disease. Take the Test, to determine if you are at risk for Type 2 diabetes.
Our program in Diabetes Self-Management Education/ Training (DSME/T) will empower the diabetic and or caregiver with the necessary training, information and skills to effectively manage and control diabetes. The program is Nationally Accredited and covers 7 key components.
We are a provider of the National Diabetes Prevention Program (NDPP). This is a Center for Disease Control and Prevention (CDC) led evidence-based program that facilitates participants in making simple lifestyle changes to prevent Type 2 diabetes. Our Training Center has met the program standards to merit Full Recognition by the CDC.
International Pre-Diabetes Center Inc. (IPDC) with divisions for Diabetes and Pre-Diabetes Professional Training was created by a Clinical Pharmacist in 2013. Our programs are structured around the realization that prevalence and risks for diabetes are driven by a complex array of social, economic, cultural and educational factors, collectively referred to as Determinants of Health (DOH). Poor health outcomes for diabetes are linked to gaps in DOH and require communities working together to address these gaps in order to prevent and control diabetes. Our Clinical Staff includes Pharmacists who are medication experts. The knowledge and support that they provide for medications combined with disease self-management and prevention education are particularly beneficial to participants in our programs. We create programs that fit the cultural, language, and un-met needs of "high risk" vulnerable populations in local communities. We have met the required program, and professional standards to merit the highest recognitions: Certificate of Accreditation by the American Association of Diabetes Educators and Certificate of Full Recognition by the Center for Disease Control and Prevention.
